I do not know / can not even make the appeal to my ban.
I started to have slow attacks and after several seconds I saw that I had knocked down several, I stopped to write that it was "Whitebug" and to avoid this, I was banned "by health".

Have you tried using the ban appeal link?

Literally put in your username you would log onto the server with and follow the instructions.

When you say "not found" can you please provide a screenshot of the issue you're having so we can assist you further.
My reaction when people type "HACKS" as a reason when reporting someone without being (Even slightly) specific about what mod/cheat they're using.

Please also post the screenshot with the URL included in it, just to be sure the correct information is being entered, as Redirect_Left said, i can also locate the appeal just fine.

Have you had a new computer and or ISP that could of changed your IP?

It looks like your IP has changed frequently according to the logs, the IP in the URL is not the same IP that you were banned under so try removing the IP in the field and simply keep the "Username" field occupied only with your game nick.
